I would like to apply for Certification/Authentication/Verification (CAV) of scholastic record. What are the requirements?

f you are a graduate or an undergraduate of any TVET programs registered under TESDA, you may apply for Certification/Authentication/Verification (CAV) at the TESDA Provincial/District Office where the school you have graduated from is located.  Bring the following requirements:

  1. Transcript of Records - Original and two (2) certified photocopy
  2. Diploma - Original and two (2) certified photocopy
  3. Special Order - three (3) sets certified true copy
  4. 2 pcs. 2"x2" latest photo in white background
  5. CAV processing fee of PhP30.00
